Cost for services and products are due in full at the time of service. I accept cash, check, or credit card (Visa, Mastercard and debit cards) for payment. Any checks returned due to insufficient funds will incur a $35.00 fee.
I am considered an “out-of-network” provider for all insurance companies. In Washington State, all insurance plans are legally obligated to cover naturopathic care to some degree. Many insurance companies will reimburse their customers for the full or partial cost of a visit with an out-of-network provider. Please check with your insurance company to determine the coverage your insurance offers for out-of-network naturopathic care.
It is your responsibility to find out what coverage your policy offers. To assist you in this process, I have created this insurance worksheet (see below). If you plan to seek reimbursement from your insurance company, please complete this form and bring it to your visit. I will provide you with a superbill which you can submit to your insurance company to request reimbursement.
I require 24 hours notice for cancellation or rescheduled visits. Any visits cancelled with less than 24 hours notice are subject to a $50.00 cancellation fee. If no notice is received, the full fee for the visit will be charged. There is no charge for any visits cancelled or rescheduled with more than 24 hours notice.
I dispense physician-grade nutritional, herbal, and homeopathic supplements. I dispense only products that I feel are of the optimal grade and quality for my patients. They are considered prescription items and are therefore only available to patients. Insurance does not cover these items. However, many patients will utilize their pre-tax health savings account (HSA) or flexible spending account to pay for these items.
Mailing of items from our dispensary are subject to a $5.00 handling fee plus postage. Payment is required prior to shipping these items.
All dispensary items must be paid for in full at the time of purchase. Items may be returned (for dispensary credit only) within 14 days of purchase, provided that they are unopened and in perfect condition. Returns will not be accepted more than 14 days after purchase. Injectables, injectable supplies, refrigerated products, products packaged in the clinic, and birth control devices cannot be returned.
